Clinical Charting: SOAP Notes for Therapy & Wellness, with Sign, Lock, Co-sign and Addenda

What it does

Clinical Charting is an Odoo module for therapy, wellness and allied-health practices that write SOAP notes. Subjective, objective, assessment and plan are separate fields. Signing a chart locks them and records who signed and when, so any later correction has to be added as a dated addendum. A clickable musculoskeletal body map holds the problem list, and a board counts what is still unsigned.

The problem it solves

An unsigned note is a draft that looks like a record. It reads the same in the file, says the same things, and carries none of the weight, because nobody has attested to it. Practices find this out in a batch. Someone runs a report at month end, finds a pile of open notes, and the clinicians who wrote them are reconstructing visits from memory. What would have prevented it is small and boring: how many are open right now, and how old the oldest one is.

What comes in the module

  1. 01

    A signed chart

    A chart moves from Draft to Signed once. Signing stamps the signer and the moment as readonly fields, freezes the four SOAP fields the template laid out, and leaves Add Addendum as the only way back in. After that nobody edits it, the author and the administrator included.

    Body zones on the chart

    Two anatomical plates behind a Front and Back toggle, with clickable zones from deltoid to erector spinae. Selecting a zone lists every problem on it, and charting a new one is a button. Findings are attached to a zone, so the same complaint is searchable across patients.

How it plays out day to day

  1. A note written on Monday is still a draft on Thursday

    The visit happened and the SOAP fields were typed, but nobody pressed Sign. Nothing on the chart says how long it has been sitting there.

    Turnaround is measured from the service datetime to the signature and stored on the chart, so you can see how long a note has been left open. The delinquent tile counts anything past 72 hours and opens those charts.

  2. Years of problems arrive as a list nobody can read at a glance

    A patient's history is ordered by date, and the clinician wants to know what is currently active on one leg.

    Each problem carries a muscle or spinal segment, a side, an onset date and a status. The Front and Back plates paint the charted zones, each side of the toggle keeps its own count, and selecting a zone lists what sits on it.

  3. The whole visit gets typed into the first box on the form

    Subjective is where the cursor lands, so the assessment ends up buried mid-paragraph and a colleague reading it back six months later has to hunt for it.

    Picking a note template lands prepared wording in all four fields, so each of subjective, objective, assessment and plan starts with something in it. Four chart types cover SOAP, progress, intake and discharge.

What it won't do

No coding and no claims
Problems carry a name, a region and a status. There is no diagnosis coding field and nothing bills.
No e-signature certificate
Signing records the Odoo user and the moment. It is not a cryptographic signature and does not claim to be one.
No HL7 or FHIR exchange
Charts stay in Odoo. Nothing here speaks to an external EMR.
No scheduling
A chart references a patient and a practitioner. Booking the visit is somebody else's model.
Body zones are fixed in code
The anatomical zones ship as a fixed list inside the module. Adding a zone to the plates or renaming one is a code change, so the interface has no screen for it.

Common questions

Can I install this on Odoo Online?
No. Odoo Online cannot install modules that contain Python, so this runs on Odoo.sh or a self-hosted instance. Check the versions listed above for the series and edition it is verified against.
Is a signed chart a legally valid electronic signature?
No. Signing records the Odoo user who pressed the button and the moment they pressed it, then locks the four clinical fields. No certificate or cryptography sits behind it. You get a record of who signed, plus a lock, and that falls short of a qualified electronic signature. If your jurisdiction requires a certified signature, this module does not provide one.
Does it code diagnoses or bill for the visit?
No. A problem carries a name, a body region, a side, an onset date and a status, and that is the whole of it. There is no diagnosis coding field, no fee, no invoice and no link to a journal entry. Coded diagnoses live in our Medical Practice module instead.
Who can reach the note templates?
Two groups ship with it. Clinical Charting User writes charts and signs them, while Clinical Charting Manager also reaches templates and configuration. Record rules fence charts and note templates by company, so a multi-company database keeps each company's charts to itself.
Can we change the wording a template puts in a chart?
Yes. A template is an ordinary Odoo record holding the starting wording for each of the four fields, so changing the house version of the intake note is an edit in the interface. No developer, no upgrade. The next intake chart opens with the new text, and charts already signed keep what was signed.
What do the dashboard tiles show, and do they click through?
Signed charts, drafts still open, active problems, charts signed within 24 hours of the encounter, and the count sitting past the 72 hour deadline. Click any tile and it opens the charts behind that number, filtered the same way, so the board hands you a list to work through.
